Step 1
The first step when creating a fillable form in Google Docs is opening a new blank document.

You can add blank underlined spaces in your sheet to indicate to viewers that they must fill that line with relevant information.

You can create underlined spaces by typing multiple underscores (‘_’) in a row.

You can also add checkboxes to your form to enable users to mark specific options in the provided space.

You can add a checklist by selecting Format > Bullets & numbering > Checklist menu.

Another way to create fillable forms in Google Docs is by adding a table. Tables allow you to collect information from your respondents in an organized manner.
To add a table to your document, select Insert > Table and use your cursor to adjust the size of your table in the provided grid.
